*  The session will not begin without the signing of this Agreement/waiver by the horse owner.

** Although it is not mandatory to fill out the Horse Information Form before the session, it is recommended to ensure the process progresses in a healthier and more efficient manner.

Filling out the form ensures that important information regarding the horse’s health, behavior, and performance history is recorded. This allows for the preparation of necessary information in advance, making the session process quicker, more effective, and more efficient. The details provided in the form (e.g., special health conditions, allergies, behavioral sensitivities) will be used to protect the horse’s health and create a safe environment. The form serves as a reference for future evaluations or situations that may arise. The information shared in the form will remain confidential between the horse owner and the practitioner.

*** Please make sure to provide the necessary information for your invoice.

**** There are no package sales. The recommended session frequency is determined based on the horse’s exercise program, health status, and individual needs. Since every horse has different needs, the session planning is tailored specifically after an evaluation.
